Talk to Queen of Arigon in the South East region of home.
Speak to Effigy outside the Tower of Life (Ardougne Teleport > South)
Go to the bridge South of the Slayer Tower and speak to the Half-Soulless (Dungeon Teleports > Slayer Tower > South)
Go to the Witch's Hut next to the Slayer Tower, seach the box inside the house, then dig outside under the eastern window
Talk to Effigy and go through the maze (Bring prayer potions and food) and click on the Caged Skeleton in the far North Eastern section of the maze. Warning: If the mask is not worn, then after 3 minutes your insanity will build up and will instantly kill you. This will result in a HCIM death. Teleport before 3 minutes are up if you are a HCIM.
Go back to Effigy and go through the maze again with the mask on, this time kill an Elite Black Demon in the end section where you got the key.
Kill the boss. It's highly recommended to use Protect from Ranged and attack the boss with Ranged while staying outside of melee distance.
